چکیده

Background The blood flow through aortic coarctations can undergo transition to turbulence, as the narrowing causes the velocity to increase. Increased levels of turbulent kinetic energy (TKE) are undesirable, as energy is drawn from the mean flow to the turbulent fluctuations. It has recently been shown that TKE in blood flow can be measured with MRI [1], and the aim of this study is to assess the turbulent kinetic energy before and after catheter intervention. Simulations of the flow were performed using computational fluid dynamics (CFD) and compared with MRI measurements.

Numerical and experimental assessment of turbulent kinetic energy in an aortic coarctation.

The turbulent blood flow through an aortic coarctation in a 63-year old female patient was studied experimentally using magnetic resonance imaging (MRI), and numerically using computational fluid dynamics (CFD), before and after catheter intervention. Turbulent kinetic energy (TKE) was computed in the numerical model using large eddy simulation and compared with direct in vivo MRI measurements....

Including aortic valve morphology in computational fluid dynamics simulations: initial findings and application to aortic coarctation.

Computational fluid dynamics (CFD) simulations quantifying thoracic aortic flow patterns have not included disturbances from the aortic valve (AoV). 80% of patients with aortic coarctation (CoA) have a bicuspid aortic valve (BAV) which may cause adverse flow patterns contributing to morbidity. Our objectives were to develop a method to account for the AoV in CFD simulations, and quantify its im...

Pressure gradient prediction in aortic coarctation using a computational-fluid-dynamics model: validation against invasive pressure catheterization at rest and pharmacological stress

Background Aortic Coarctation (AoCo) accounts for 5-8% of the children with CHD. Even after successful early repair, life expectancy is still markedly reduced (80% at 50 years after surgery) compared to normal population due to long term complications (hypertension). Usually, invasive diagnostic catheter investigations are required to evaluate the pressure gradient across the aorta at rest, or ...

Beyond Pressure Gradients: The Effects of Intervention on Heart Power in Aortic Coarctation

BACKGROUND In aortic coarctation, current guidelines recommend reducing pressure gradients that exceed given thresholds. From a physiological standpoint this should ideally improve the energy expenditure of the heart and thus prevent long term organ damage. OBJECTIVES The aim was to assess the effects of interventional treatment on external and internal heart power (EHP, IHP) in patients with...

Three Different Imaging Modalities of a Patient with the Aortic Coarctation

The patient was a 19 year-old woman with the diagnosis of resistant hypertension, although she was under treatment of three classes of anti-hypertensive drugs (beta blocker, angiotensin receptor blocker, diuretic) for more than one year. In physical examination there was only a significant difference between the systolic blood pressure of upper and lower extremities (200 vs. 120 mmHg), without ...
